Available Survey Templates
Survey templates make it easier to collect feedback from employees
When creating a Custom Survey in goHappy, you can choose from a collection of pre-made templates instead of starting from scratch. Each template comes with pre-built questions that you can customize to fit your organization's needs.
How to Access Templates
- From the Dashboard, click Create a Survey — or navigate to Feedback > Custom Surveys and click Create a Survey
- Select Use a Template
- Browse the available templates and click the one that fits your need
- Customize the questions, title, and settings as needed before launching
Available Templates

| Benefits | A template to gather employee feedback on their benefits package |
| Question Submission | A template to collect questions in advance of a meeting |
| Suggestion Box | A template to gather employee suggestions and gauge interest in helping implement new ideas |
| Contest | A template to gather input on contest ideas and suggestions |
| Icebreaker Game | A template to spark fun and connection through lighthearted team questions and personal sharing |
| Giveaway | A template to collect giveaway submissions |
| Training | A template to gather feedback on training sessions |
| Contact Info | A template to collect basic contact details for each employee |
| RSVP | A template to help you start collecting RSVPs for your event. If you have smart reply, instead this could be done with a text, see instructions here |
| Communication Survey | A template to ask your team what updates they want to see more of |
| eNPS | Collect eNPS (Employee Net Promoter Score) from your employees |
Tips for Using Templates
- Templates are a starting point — all questions can be edited, added to, or removed after selecting a template
- You can copy an existing survey if you've already customized a template and want to reuse it for a future send
