The items below are designed to help you assess your effectiveness in carrying out the Leader Action - Coach.  Review your current performance against each item and  rate the frequency with which you carry it out in accordance with the following scale: 

1- Never; 2-Rarely;  3 -Sometimes; 4-Often;  5-Always
